Step 1 - Online Registration
Registration will be open from Monday, June 2 through Thursday, June 26. If you have any questions regarding registration for the 2025-2026 school year, please contact us at 847-327-7700.
New Students who have just moved into our district, please contact Christi Steiner, Transition Pathways Supervisor at 847-327-7701 for more information.
First Day of the 2025-2026 School Year is August 11, 2025.
Navigate to our Registration Gateway System to begin
Returning D128 students, your login information will be mailed to your homes the first week of June. If you have any issues logging in, contact us at 847-327-7700 and we can assist you.
- Once you log in, click Edit to begin.
- Scroll through each page and make sure that all information that we have on file is still current and update any information that is needed. Some information such as medical is required to be updated each year.
- Make sure ALL parents are listed as contacts, even if they do not live in the same household. It is extremely important we have all contact information on file.
- If you have changed addresses, update the information AND complete this form with proof of residency documentation. Registration will not be considered complete until both steps are done.
Step 2 - PowerSchool Account
If you already have an active PowerSchool account for all the students you are registering, you do not need to do ANY of the steps on this page.

If you have an active account but need to link a NEW student to your account:
You need the Parent Access ID and Access Password for each student you want to link to your Single Sign-on. Please contact our office at 847-327-7700 if you do not have this information.
If you are new to the district and do not have an active PowerSchool account, you would need to add one:
- A parent/guardian new to District 128 needs to create their own Power School account. (A PowerSchool account from another school cannot be used for District 128.)
- Navigate to the D128 Parent PowerSchool Portal to create your account!
- Create a parent/guardian account by entering the information below unless you have an active account with a current student in District 128. If so, sign in and skip to "add students" to your existing D128 parent account below.
Your First and Last Name

- Choose a username and password (please note that usernames and passwords are case sensitive)
Step 3 - Pay Registration Fees
Log into your parent PowerSchool account and choose Balance from the left hand menu. Once your student's fees have been displayed, you may click on Make a Payment in the upper right of your screen and you will be redirected to the webstore to pay your fees.
You will need to set up a web store account if you are a new parent to LHS or if you have not previously used the web store.

- Registration Fee $325
- The activity pass is being removed. In its place, all D128 students may attend home athletics and many fine arts activities at no additional fee. Some events (IHSA playoffs, musicals, festivals, etc.) will still require an admission fee.
If you are unable to pay your registration fees, please complete the online Basic Fee Waiver application or print and complete the paper Basic Fee Waiver application for each student in your family. Please make sure if filling out the online form, when you get to the portion to upload the documents, remember to click Send Files after uploading. You will then need to return to the form to submit the rest of the information.

Step 4 - Proof of Residency
We digitally verify residency for all new incoming District 128 families.
Only the following situations need to provide additional documentation to verify proof of residency:
- Incoming families who we were not able to digitally verify residency. These families will be notified in the registration letter that is was sent out and will need to complete this form and upload the documentation.
- Current District 128 Parents/guardians who have moved/changed their address must re-verify by completing this form AND uploading the documentation.
All proof of residency forms need to be complete by June 27.
Your registration is finished if you have:
- Completed your student's online registration
- Created a Parent PowerSchool Account (if you didn't already have an existing account)
- Paid your registration fees via our web store
- Provided Proof of Residency if required
- Students new to District 128 must submit health, dental forms if you haven't already done so! Documents are linked in Part 6.

Step 5 - Medical Forms Submission
All students new to District 128 must have a complete ISBE health physical, dental exam & immunization record on file prior to the first day of school. In addition, any student wishing to play a sport, needs a sports physical on file. Medical forms including physicals, seizure action plans, administration of medication etc can be submitted the following ways:
